Notice of Non-discrimination Admission: CCA will admit students of both sexes regardless of race, creed, color or national origin, until the enrollment limit has been reached. Both student and parents must agree to abide by the Student (In the fourth grade and above) Parent Handbook of CCA and function appropriately within the school environment.
Enrollment Policy: Application forms are to be filled out completely by a parent or guardian and turned into the administrative office with the registration fee, which is non-refundable. An interview may be scheduled with prospective students for grades K4-8. The administrator, a grade level teacher (when possible), and board member will meet with the student and parent to get better acquainted, explain policies of C.C.A., and share information about procedures which would enable the school to best meet the needs of the student and family. Once the admissions process is completed, the application information will be reviewed. The central office will promptly notify the parents regarding the admissions request. Upon acceptance into the academy, applicants will need to furnish the following:
- Certified Copy of Birth Certificate
- Copy of Social Security Card
- Updated Immunization Certificate
- Health & Emergency Form
- Application for Admission
- Complete Eye Exam (Not only a screening) (Those entering Preschool and again in Kindergarten)
- Parent/Student Agreement Form (Signed by all parents and students grades six and up)
- Filed Trip Permission Form
- Request for Student Records (If transferring from another school).
- If applicable, results of psychological or other special testing are also required.
- Students in grades six and up and their parents must sign a "CCA Computer Policy" .
- Students entering grade six must have proof of a current health physical and immunizations.
